Intel CTR-21 Driver. This is for a PCI "Ham" modem marked with Intel model CTR-21. Need to peel the Intel lable off of the chip to make sure this matches yours, but this driver is for the Ambient MD5628D-L-C chipset. Is on Intel.com, but tough to find!!
